Bidder/Contractor Questionnaires

The Contractor Responsibility Ordinance (CRO) requires bidders/proposers for a City contract to complete a Responsibility Questionnaire. Posted on this website are the responses from the three lowest bidders for competitive bids, and responses from the firms short-listed for Requests for Proposals/Qualifications. Questionnaires from proposed contractors selected through a sole-source process are also posted. The posted Questionnaires will remain on the City website for 14 calendar days.

The Questionnaires are located on three separate webpages, depending upon the type of contract to be awarded: Construction Contracts, Procurement Contracts, and Service Contracts. All three webpages should be reviewed to determine if a bidder/proposer is listed on more than one webpage because the bidder may be submitting bids for more than one type of contract. The Questionnaires listed on each page are arranged according to the name of the contract, with the most recent Request For Bids/Proposals/Qualifications at the top of the page. Complaints regarding a Bidder's/Proposer's or Contractor's responsibility may be forwarded to the City department that has been designated as the administrative agency (DAA) responsible for administration of the Contractor Responsibility Program. Depending upon the type of contract, one of three DAAs may be contacted: Department of Public Works for construction contracts; Department of General Services for procurement contracts; and Office of the City Administrative Officer for service contracts. Contact information for each DAA is available on the individual web pages.

Debarred Bidders/Contractors

A listing of bidders/proposers and contractors who have been debarred from bidding and contracting with the City of Los Angeles is available through the link provided below. The list is maintained by the Office of the City Administrative Officer (CAO) and compiled based on information submitted by City departments that have debarred a bidder/proposer or contractor.
