To report an injury, damage or loss to personal property, contact Los Angeles Convention Centers Security Central at (213) 741-1151, ext. 4605 or 5900 to file an Incident/Loss Report. For any questions regarding filing a claim, or to receive a copy of Incident/Loss Reports, contact the Administrative Services Division at (213) 741-1151, ext. 5320. All claims against the Los Angeles Convention Center must be filed with the City Clerk Office. To receive a copy of the Claim for Damages Form, contact the City Clerk Office at (213) 978-1133. You may also download the Claim for Damages Form from www.lacity.org/clk/. The Claim for Damages Form must be completed, signed, and sent to the Office of the City Clerk at the following address: Office of the City Clerk, 200 N Spring St., Room 395, City Hall, Los Angeles, CA 90012.
