Disease Prevention and Health Promotion
The Partners in Care Foundation under contract with the
Department of Aging, administers Disease Prevention and
Health Promotion services to older persons. The program
is led by a collaborative of medical and health professionals
working directly with the Department's 16 Multipurpose Senior
Centers and many congregate nutrition sites.
Under this program, City seniors are provided free health screenings
(such as glucose, vision, hearing, gait and balance, memory loss,
movement disorders, and others), health education, counseling and
referrals. The program also provides opportunities to medical
residents, advanced nursing, pharmacology, social work, and
gerontology students to work with older adults in their chosen
areas of study.
